Safe systems need continual checking. This is to ensure reliability of operation. If a safe system is working successfully, do not presume it always will. Just like any piece of machinery, there needs to be continual input of energy to keep everything working. Just like an engine needs oil and oil checks, so do safe systems

Using a checklist, helps identify any issues that may put user at risk. Regular checks can pinpoint changes that otherwise may not be spotted.

Checklist are used to to comply with specific legal requirements. there are some specific laws that require regular checking ..egs..

Ensure suitability for task to be performed.

Check adequacy of protective devices provided. All protective clothing needs regular checking to see how good the fit is, and whether wear and tear has rendered it useless.

6 % of sky divers that are killed do so because they forget to pull the cord. They forget - not because they are inexperienced, as everybody makes sure for the first few times. It is only after a while that you can't remember whether you pulled the cord this time or last time, or the time before. That is why a pilot has a checklist on take off and landing and doesn't rely on memory.
